Please plan to arrive one half hour prior to your appointment so information forms can be completed prior to seeing the doctor.
Please bring a family member or close friend who also can talk with the doctor about your memory problem.
You will be asked to make a follow up appointments for further tests, to go over the results of your examination and to discuss treatment options.
Please bring your medical records with you or have them faxed to us.
